Preventative Health

PivotWest aims to assist and develop GP skills by implementing a pro-active approach to Preventative Health, with a specific focus on reducing the rate of chronic disease. We provide support and advice to GP’s in the identification of ‘at risk’ patients and implement early intervention strategies wherever possible.  PivotWest also promotes government initiatives, undertaking small local projects as well as offering a number of education events centering around Preventative Health and Chronic Disease Management throughout the west of Melbourne.

  • 45 – 49 Year Old Health Check
    -
    The health check is targeted at people who are between 45 and 49 years of age (inclusive) who are at risk of developing a chronic disease. Risk factors might include:
    -
    lifestyle risk factors, such as smoking, physical inactivity, poor nutrition or alcohol misuse
    -
    biomedical risk factors, such as high cholesterol, high blood pressure, impaired glucose metabolism or excess weight
    -
    family history of a chronic disease
